Holacracy Directory Form

Thank you for applying for a Holacracy Badge!

This form serves two purposes:

  1. To collect information for your public company profile on holacracy.org
  2. To help us understand your Holacracy practice and determine badge eligibility (including a short self-assessment)

Some of your answers will be published on your public profile—others will remain private. By default, responses are public unless stated otherwise in the question or section intro. When in doubt, please read the guidance provided on each page.

The form is divided into short sections and should take about 10–15 minutes to complete.

We recommend using a computer (not a phone or tablet).

You'll need:

  • Your company logo
  • A screenshot of your org chart
  • A team photo (optional)

Once submitted, we’ll review your application and follow up within about a week.

To get started, please enter your individual contact info so we know who to reach if we have questions. No generic company emails here please (we'll ask for that one later).

This first section collects basic information about your company. All responses will be published on your public profile in the Holacracy directory.

Some tools (e.g. GlassFrog) allow you to make your organizational chart public. This allows you to showcase your organization to anyone on the Internet. If you have made your organizational chart public, please enter it here. People love to see examples of Holacracy "in real life."

Your Holacracy Practice

This section explores how deeply you've adopted and integrated Holacracy, and what kind of support or training you’ve received along the way. Just answer based on your current experience—there’s no right or wrong.

 


How deeply are the Articles of the Holacracy Constitution embedded in your everyday operations?

As a reminder, the Holacracy framework is defined by the Holacracy Constitution—the formal set of rules that guide how the system works. It’s divided into five distinct Articles, each of which can be adopted to varying degrees and even independently of the others.

For reference, the Articles are:

  1. Article I: Organizational Structure
  2. Article II: Rules of Cooperation
  3. Article III: Tactical Meetings
  4. Article IV: Distributed Authority
  5. Article V: Governance Process

This question helps us understand how much of the framework your team has put into practice.
